A palliative care doctor and nurse visit the patient’s home to conduct a full medical and psychosocial assessment.
Nurses update the patient’s digital health record, which is synced with our central system & doing their daily works.
The RPMS continuously tracks vital signs and alerts our medical team, Data is reviewed by our clinical team daily to adjust care plans as needed.
Any changes in the patient’s condition trigger coordinated action—whether it’s a nurse visit, a teleconsultation, or a hospital referral.
Senior Nurses are able to perform both nurse and patient personal caregiving, whereas Junior Nurse are able to perform patient personal caregiving only.
